I understand that no Partners in Education volunteer is eligible to volunteer at a school site until they have been authorized to do so by Santa Barbara Partners in Education and all screening requirements have been fulfilled. I also understand that it is my responsibility to communicate any information related to my requested volunteer activity to other school staff, according to any requirements my own site may have.

I understand that the success of volunteer recruitment for my request is partially dependent upon my ability to accommodate volunteer schedules and to reply to Partners in Education communications in a timely manner and that my volunteer request is subject for review by Partners in Education prior to being approved for recruitment and web posting.

I certify that the information provided on my request form is true and correct to the best of my knowledge. I understand that any false or misleading statements of material may subject me to disqualification.

By submitting this application, I hereby release Santa Barbara Partners in Education and its officers, directors, employees, sub-contractors, sponsors, agents and affiliates from any and all liability relating to instances that may occur during volunteer activites or Partners in Education events and from all present and future claims that may be made by me, my family, estate, heirs, or assigns for property damage, personal injury, or wrongful death arising as a result of my participation in the volunteer activities wherever, whenever, or however the same may occur.

This request is for volunteers only and does not apply to paid employees, temporary employees, substitues or ther employees. Should a volunteer engages in service with SBCEO/PARTNERS choose to become a paid employee or contractor with a volunteer site, that employee or contractor and the volunteer site will be responsible for following all applicable screening and background checks as required by governing policies and state and federal law.
