I understand that I am not eligible to volunteer at a school site until I have been authorized to do so by Santa Barbara Partners in Education and all screening requirements have been fulfilled.
On occasion, representatives from and/or employees of the Santa Barbara County Education Office wish to photograph, videotape, and-or interview individuals in connection with school programs, projects, or events. Similarly Santa Barbara County Education Office wishes to display students’ creative works
With respect to all photos, videotaping, audio recordings, creative student works, and reproductions of same in any medium including SBCEO web sites, I hereby consent to and authorize their use by SBCEO for reproduction, distribution, and exhibition for any purpose and in any medium whatsoever including (but not limited to) publication, website, broadcast, display, and exhibition for educational purposes, without compensation or notice to me.
I agree that I shall have no right, title, or interest in any photo or videotape covered by this agreement and waive any right to compensation for such use.
I certify that the information provided on my volunteer application is true and correct to the best of my knowledge. I understand that any false or misleading statements of material may subject me to disqualification or dismissal.
By submitting this application, I hereby release Santa Barbara Partners in Education and its officers, directors, employees, sub-contractors, sponsors, agents and affiliates from any and all liability relating to instances that may occur during volunteer activities or Partners in Education events and from all present and future claims that may be made by me, my family, estate, heirs or assigns for property damage, personal injury, or wrongful death arising as a result of my participation in the volunteer activities wherever, whenever, or however the same may occur.
